UF Health primary care centers in Jacksonville offer services for patients during every stage of life, from pediatrics to geriatrics. University of Florida physicians work with physician assistants and advanced practice nurses in primary care centers throughout Northeast Florida and St. Marys, Georgia.
Patient care is our top priority, so our offices offer appointments within 24 hours. Services in many of our centers include annual checkups, well-baby visits, sports physicals and disease prevention, as well as management of chronic medical conditions such as high blood pressure, diabetes and depression. Online appointments are also available through our UF Health Virtual Visit program.
Because we are affiliated with the University of Florida, patients have access to all of the benefits of an academic health center, including advanced medical treatment from UF specialists as well as the latest in cutting-edge research.
UF Health Jacksonville is an HEI leader for the unique needs of our LGBTQ patients. We provide comprehensive care through coordinated services lines, from family medicine to specialty care, and support through community resources.
Licensed clinical social workers, or LCSWs, are available to see patients in a primary care setting at UF Health Family Medicine practices and through UF Health Virtual Visit. They assess, diagnose and treat behavioral health conditions, coordinate care services and offer individual and family counseling.

The National Committee for Quality Assurance (NCQA) has designated UF Health Jacksonville primary care centers as Patient-Centered Medical Home providers. UF Health Jacksonville is the first health organization in Northeast Florida as well as the largest academic medical group in Florida to receive this designation. NCQA describes the Patient-Centered Medical Home as a model for care were providers and patients, family members and caregivers partner together to achieve better outcomes.
