Your medical record is an important part of your health care experience. You have several options to making a request for your medical records. If you have any questions, please call the Health Information Management Department at (904) 244-2596 or (904) 244-2597.
Generally speaking, face masks are not required in UF Health facilities. However, to ensure the health and safety of our patients, visitors and employees, properly worn face masks will be required under special circumstances.
